Dear Residents

We have been provided with the following information:

Integrated Care Partnership Strategy

The Integrated Care Partnership is developing a five-year strategy that will set out how organisations such as health and social care, councils, police, fire and the voluntary and community sector could work with communities to improve the health and wellbeing of the local population from birth to end of life.

The strategy aims to identify opportunities where the outcomes and experience of residents could be improved through partnership working and will focus on five key areas:

  • People and Communities – working with people and communities to empower them to build healthy, supportive and thriving neighbourhoods.

    • Prevention and Inequalities – helping people to make healthier lifestyle choices and to support people staying well for as long as possible. Making sure that everyone has the same opportunities to access services and information.

    • Personalised Care – making sure that when people do need care and support, it is tailored to meet their individual needs and preferences. Working together to make services more joined up.

  • Personal Responsibility – supporting and enabling people to make healthier choices and to be responsible for managing their own health and wellbeing.

    • Productivity – reducing waste and making the best use of resources. Targeting services to those in greatest need and making services more joined up and efficient.

To support the development of the strategy, we have reviewed feedback received through various engagement exercises that have been undertaken by ICP partners over the last few years and have been out to community groups to help shape our collective priorities.

We’ve been asking people what they think would help communities to live healthier lives, what are some of the issues that can sometimes get in the way, and what more we could all do to improve the health and wellbeing of Staffordshire and Stoke-on-Trent over the next five years and beyond.

To continue this conversation, the ICP is holding an additional on-line focus group on Wednesday 10th May, 11am – 1pm and would like to invite all councillors from across Staffordshire to attend. The aim of the focus group will be to provide an update on the ICP strategy and to hear your views about what would support the constituents you represent in your local area.

To register, please use the link and a team’s invite will be sent out via email.

A further focus group will be open for members of the public to attend from across Staffordshire and Stoke-on-Trent. This will be held on Wednesday 17th May, between 1pm – 3pm. To register for the public event, please use the link
